Since many messages will be part of a greater overall piece of functionality, use the # of messages completed to calculate a rough estimate of % completed.

Implement message timeout stability pattern

When sending a message, allow specification of a timeout at the handler, and override when sending a message.   On timeout, a timeout message is send, which can be handled as the consumer wishes.

Allow handler setup to be able to specify if handlers are reentrant (the default) or serialized.
